Step Four

• Complete the Registration Process by following the steps below. Choose the school that you're attending.
  

                                     High School Activity Registration Form

                                     Middle School Activity Registration Form

• Complete the Student and Parent Guardian information
•  Submit your completed and signed Physical Card
• Complete the Emergency Card Primary Care and Health Information
• Date when you have read the rules and documentation of the registration process
• Submit your signed online agreement stating the student and parent guardian have correctly provided the needed personal information and have read, understand and agree to follow all the rules and documentation provided in the four steps on this  website.
