Step Three

Step Three: Obtain and prepare to submit your signed Physical Card.
 
All students must have a physical examination every two years if they are involved in any activity other than a "Group C" Activity:
A physical examination taken April 1 and thereafter is valid for the following two school years; a physical examination taken before April 1 is valid only for the remainder of that school year and the following year.
Example:

A physical examination taken April 1, 2022 and thereafter is valid for the following two school years: 2022/2023 and 2023/2024. 

A physical examination taken before April 1, 2022 is valid only for the remainder of the 2021/2022 school year and the following 2022/2023 school year.

If your student's physical was before April 1, 2022, they will need a new physical for the 2023-2024 School Year

* Try to schedule your student's physical after April 1st and before the start of the upcoming school year.*

 

Athletes/Students  WILL NOT be allowed to participate without having a current physical exam on file.

All documents in regards to the Athletic Physical should be submitted to the School's Activity Department. You can print a copy of the Physical Examination card below.

UPLOAD A SCANNED COPY OF THE COMPLETED PHYSICAL FORM in Step Four.
Please make sure that the students name, date of the exam and a parent's signature are all on the physical form before uploading.
If that is not possible: please send, drop off or bring your completed Physical Examination card with you to your Sports Meeting or Co-Curricular Office at your school.
